Emanuel Yaakobov is a board-certified family nurse practitioner who joined Future Care Internal Medicine in 2026. He is certified by the American Nurses Credentialing Center.
He earned his Master of Science in Nursing at Long Island University C.W. Post and his Bachelor of Science in Nursing at Adelphi University. Before joining the practice, he built acute-care experience in the emergency departments at Elmhurst Hospital Center and Forest Hills Hospital and in home care across Queens and Long Island. His focus at the practice includes management of acute and chronic disease.
Ilana Levich is a Board-Certified Family Nurse Practitioner dedicated to providing compassionate, patient-centered care to individuals and families across all stages of life. She earned her Bachelor of Science in Nursing from Wagner College in 2019 and began her nursing career at Memorial Sloan Kettering Cancer Center, where she worked as a Registered Nurse in the Emergency Department from 2019 to 2024. During her time at Memorial Sloan Kettering, she cared for patients with complex and acute medical conditions while gaining invaluable experience in emergency and oncology care.
While working full-time as a Registered Nurse, Ilana pursued her advanced education at Wagner College, earning her Master of Science in Nursing and Family Nurse Practitioner degree in December 2023. Since May 2024, she has been practicing as a Family Nurse Practitioner at Future Care Internal Medicine, where she provides comprehensive primary care for patients of all ages.
Ilana is passionate about building lasting relationships with her patients and believes in delivering personalized, evidence-based care with empathy and respect. She manages a wide range of both acute and chronic medical conditions, focusing on disease prevention, health promotion, and improving each patient’s overall well-being.

Born in Florida, Dr. Sengupta grew up in Bloomfield Hills, MI. He attended Cornell University, majoring in Neuroscience, before moving on to the Yale School of Medicine where he earned his MD.
He is fellowship trained in interventional pain management and board certified in internal medicine. His clinical areas of focus are regenerative medicine and neuromodulation.
Dr. Sengupta is an experienced specialist in site-of-care optimization and the implementation of medical services. He has conducted groundbreaking, first-in-human clinical research using stem cell derived biologics to treat COVID-19 related ARDS and cytokine storm, as well as the use of implantable neurostimulation devices for the treatment of pain, and is currently the primary investigator on several national multi-center clinical trials.
He has served on faculty at Harvard and NYU.
